This is the public website for the C3-Cloud research & development project which aims to develop an ICT platform to support integrated care of patients with multiple long-term conditions, who are seen by several care teams. The website gives essential information about the project aims, project partners and results.

One of our partners is based in Turkey which is outside of the European Economic Area. If we send any personal data to this partner we will ensure that it is protected by the use of EC model clauses.
