The European Commission has just published its communication to the European Parliament and Council on “enabling the digital transformation of healthcare in the digital single market: empowering citizens and building a healthier society”. This report emphasises the need for digital tools for citizen empowerment and person centred care, the promotion of interoperability standards and better use of data to promote research, disease prevention and personalised care.

C3-Cloud welcomes this report, elevating to a high visibility the case we are making for better care through better information to help clinicians and empower patients dealing with multiple long-term conditions (multi morbidity) and taking multiple medications (polypharmacy). C3-Cloud is developing digital solutions to support clinicians with multi-condition care pathways and better team collaboration tools, and a patient empowerment platform to help individuals and their caregivers to play a more active role in self management of their conditions and in prevention.
