C3-Cloud will be present at the Informatics for Health Congress 2017, to be held in Manchester, UK between the 24th and 26th April 2017. The Congress brings together Europe’s leading conference, Medical Informatics Europe (MIE2017) from the European Federation for Medical Informatics (EFMI) and The Farr Institute International Conference 2017.
The poster “C3-Cloud: A Federated Collaborative Care & Cure Cloud Architecture for Addressing the Needs of Multi-morbidity and Managing Poly-pharmacy” will give an overview of the project. In brief, C3-Cloud aims to establish an ICT infrastructure to enable the continuous coordination of patient-centred care activities by a multidisciplinary care team and patients/informal care givers with the help of clinical decision support modules. This approach considers some of the shortcomings in the care provision of multi-morbid patients that are more complex, disconnected and time-consuming than those with single diseases. C3-Cloud aims to deliver a collaborative tool enabling reconciliation of multiple care plans targeting different conditions of the patient and with the support of decision support aids aims to prevent poly-pharmacy.
